Attached to the Supplementary Information Form (SIF) must be photocopies of your child's Baptism, Birth Certificate and 2 proofs of Residency (bill must be dated within the last three months).

RECEPTION ADMISSIONS
FOR SEPTEMBER 2024/2025

Please note that you must ring your Parish to make an appointment to get your form signed. There will be allocated times when your Parish Priest will be able to sign the forms. Make sure that you are aware of these times, which will be announced in your Parish newsletter.

You will be applying for a place for your child in the Reception class for September 2024. Please note that you will have to apply to the Borough of Brent at www.brent.gov.uk/admissions using the online Application Form. The form allows you to apply for six schools, and these are to be listed in order of your choice. The phone number for the office if you have any queries is 020 8937 3110.

30 HOURS NURSERY CHILDCARE
From September we will be offering 30 hours of childcare (to families who are eligible) in the Nursery. Should you need more information regarding the eligibility please ring the helpline and look at the relevant links below.

APPEALS INFORMATION - Academic Year 2024/2025
Deadline for applications is January 15th 2024 for children due to start Reception Class in September 2024.
Primary offer day for your child is 16th April 2024.
If you do not receive an offer of a place for your child from a school of your choice you have the right to appeal.
The deadline to return the appeal application is TBC.

CERTIFICATE OF CATHOLIC PRACTICE FORM TO BE SIGNED
If you are a parishioner of the English Martyrs Parish, Fr Albert will be signing the CCP form. Please look out for the dates in the Parish newsletter where you will need to phone the presbytery to make an appointment.

If you are new to the Parish, please make sure you introduce yourself to the Parish Priest.
DIOCESE OF WESTMINSTER REQUIREMENT
The Diocese of Westminster requires that your parish priest completes a Certificate of Catholic Practice. (Priest Reference Form)
The parish Priest will fill in this form which has to be returned, along with the Supplementary Information Form to the school you are applying for.
