Certificate of Catholic Practice (Priest Reference Form)
Attached to the Supplementary Information Form (SIF) must be photocopies of your child's Baptism, Birth Certificate and 2 proofs of Residency (bill must be dated within the last three months).

Covering letter for Reception
Please note that you must ring your Parish to make an appointment to get your form signed. There will be allocated times when your Parish Priest will be able to sign the forms. Make sure that you are aware of these times, which will be announced in your Parish newsletter.

You will be applying for a place for your child in the Reception class for September 2019. Please note that you will have to apply to the Borough of Brent at www.brent.gov.uk/admissions using the online Application Form. The form allows you to apply for six schools, and these are to be listed in order of your choice. The phone number for the office if you have any queries is 020 8937 3110.
APPEALS INFORMATION - Academic Year 2019/2020
Deadline for applications is January 15th 2019 for children due to start Reception Class in September 2019.
Primary offer day for your child is 18th April 2019.
If you do not receive an offer of a place for your child from a school of your choice you have the right to appeal.
The deadline to return the appeal application is 16 May 2019.
